What you'll get to do...

Your experience should include...

  • Experience in Support Coordination – Previous experience working as a support coordinator, care coordinator, or in a similar role.
  • Understanding of Care Plans – Ability to develop and manage person-centered care plans.
  • Knowledge of Social Services – Familiarity with community services, healthcare systems, and social work practices.
  • Strong Communication Skills – Excellent written and verbal communication for effective interaction with clients and teams.
  • Problem-Solving Abilities – Ability to identify challenges and find practical solutions for clients.
  • Collaboration with Healthcare Providers – Experience working alongside multidisciplinary teams to support individuals.
  • Client Advocacy Experience – Previous experience advocating for clients in complex care situations.
